Team Portrait Handout and Order Form
To order simply e-mail the info requested in the form to me at paul@idealphotography.biz . Thanks.

Finished collages will have 2010, Athlete's name, and age, plus any other info you would like.

Collages are available for all athletes that have had their portraits taken. Portraits are shown below.

8x10 collage: chose team picture and either a head shot or an action shot of your athlete.

Larger than 8x10 you may request both a head shot and an action shot or just one.
